Financial Information

At the time of your initial visit, your treatment plan will be established and an estimate of fees will be given to you. Our goal is to provide you with optimal care based on your individual needs. You can choose to pay by cash, check or major credit card, or we offer Care Credit, a lending company, a division of GE Consumer Finance.

PAYMENT IN FULL: We accept cash, check, Visa, MasterCard, American Express and Discover

CARECREDIT PAYMENT (LENDING COMPANY): If you are interested in applying for Care Credit, please do so ahead of your scheduled appointment by visiting http://carecredit.com. (If Care Credit is utilized it will be between you and Care Credit from that point forward).

INSURANCE: If you have insurance and would like our office to file your insurance, we would be happy to do so as a courtesy to you; however, we will need your insurance information the day of service. If this information is not provided, we will assume you have no insurance coverage and you should be prepared to pay your balance in full.(Your COPAY/PERCENTAGE and DEDUCTIBLE will need to be paid at the time services are rendered).

We will make every effort to accommodate your needs.

(After insurance reimburses the Kentucky Center For Oral & Maxillofacial Surgery for the services rendered to the patient, if there is a balance it should be paid within 30 days. If an account goes over 30 days there will be a finance charge of 1.5%. If your account goes over 60 days, it will be turned over to a collection agency unless other arrangements have been made.)
